• Completes ongoing assessments and updates nursing diagnoses as needed, involving family when appropriate.
• Administers medications, treatments and immunizations as ordered by the physician.
• Documents care, assessment findings, and patient responses in medical records.
• Coordinates patient referrals and collaborates with the multidisciplinary team.
• Provides patient and family education and supports discharge planning and safety.
