Job summary

Intermountain Health is seeking a Clinical Program Coordinator for the Trauma department at Good Samaritan Hospital in Lafayette, Colorado. The role guides patients through the treatment continuum, coordinates with physicians and nurses, and provides education during the hospital stay and after discharge.

Minimum qualifications include a Bachelor's in Nursing, current RN license, BLS-HCP certification, and at least three years of RN experience; master's degree and specialty certification are

Qualifications

Bachelor's Degree in Nursing
Current license as a Registered Nurse in the state of practice
Current American Heart Association Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire or within 60 days of hire

Responsibilities

Serves as a coordinator for the assigned program and ensures care meets evidence-based guidelines; educates patients and families during hospital stay and serves as a resource after discharge.
Participates in patient care rounds.
Collaborates with the director to ensure regulatory requirements are met; performs chart reviews and data analysis for performance improvement.
Collaborates with the director to develop and implement evidence-based policies and standards for the program.
