Job summary

Intermountain Health is seeking a Clinical Program Coordinator for the Trauma department at Good Samaritan Hospital in Lafayette, CO. You will assess, plan, implement, and coordinate treatment processes from diagnosis through follow-up, guiding patients through the care team and providing education.

The role requires an RN with a BSN, BLS-HCP certification, and at least three years of RN experience. Regular on-site attendance and flexibility for nights/weekends are expected.

Qualifications

• Bachelor's Degree in Nursing required.
• Current Registered Nurse license in the state of practice.
• BLS-HCP certification at hire or within 60 days of hire.
• Three years of RN experience required.

Responsibilities

• Coordinate the program, discuss with physicians and nurses, educate patients and families during hospital stay and after discharge.
• Participate in patient care rounds.
• Collaborate to ensure regulatory compliance; perform chart reviews and data analysis for performance improvement.
• Develop and implement evidence-based policies and standards for the program.
• Maintain data and QA program; serve as subject matter expert.
• Promote mission and values and perform other duties as assigned.
• May float to other departments or be on-call as needed.
