Job summary

Intermountain Healthcare Inc. is seeking a Clinical Program Coordinator for the Trauma department at Good Samaritan Hospital in Lafayette, Colorado.

The role coordinates patient care processes from diagnosis through follow-up, guiding patients through the collaborative care model with physicians and support programs. The position requires a Bachelor’s in Nursing, current RN license, and 3 years of RN experience; BLS-HCP is required on hire with a 60-day window.

Qualifications

• Bachelor's Degree in Nursing is required.
• Current license as a Registered Nurse in the state of practice.
• BLS-HCP certification on hire or within 60 days; ACLS/NRP/PALS as appropriate.
• Three years relevant RN experience.

Responsibilities

• Coordinate the assigned program and ensure care meets evidence-based guidelines.
• Educate patients and families during hospital stay and post-discharge.
• Participate in patient care rounds and collaborate with care teams.
• Develop and implement evidence-based policies and standards for the program.
• Maintain data and serve as the program's subject matter expert.
• Promote mission, vision, and values of the organization and comply with service standards.
