Job summary

Intermountain Health is seeking a Clinical Program Coordinator for the Trauma department at Good Samaritan Hospital in Lafayette, Colorado. The role centers on coordinating treatment processes, guiding patients through care with multiple providers, and delivering education through the hospital stay and after discharge.

Three years of RN experience is preferred, with BLS-HCP at hire or within 60 days. Full-time, 40 hours per week, on-site scheduling includes nights/weekends as needed.

Qualifications

• Bachelors degree in nursing required.
• Current RN license in the state of practice.
• BLS-HCP certification at hire or within 60 days of hire.

Responsibilities

• Coordinate treatment processes from diagnosis through follow-up.
• Educate patients and families during hospital stay and after discharge.
• Participate in patient care rounds and QA activities.
• Develop and implement evidence-based policies and standards for the program.
• Maintain data and serve as program subject matter expert.
