Responsibilities

• Guide patients through the collaborative process with medical care providers and support programs.
• Provide patient and family education during hospital stay and after discharge.
• Participate in patient care rounds.
• Collaborate to ensure program meets regulatory requirements and review chart data for improvement.
• Develop and implement evidence-based policies and standards for the program.
