Job summary

Intermountain Health seeks a Clinical Program Coordinator for the Trauma department at Good Samaritan Hospital in Lafayette, CO. This full-time role (40 hours/week) involves guiding patients through care with a multidisciplinary team and educating families during hospitalization.

Requires a BSN, current RN license, BLS-HCP certification, and 3 years of RN experience. On-site with a generous benefits package; on-call and occasional float to other departments may occur.

Qualifications

• Bachelor's Degree in Nursing required.
• Current RN license in the state of practice.
• Current American Heart Association BLS-HCP certification upon hire or within 60 days with approval (ACLS/NRP/PALS as appropriate).
• Three years relevant RN experience

Responsibilities

• Coordinate treatment processes from diagnosis through follow up and guide patients and families through the hospital-based care continuum.
• Actively participate in patient care rounds and ensure care meets evidence-based guidelines.
• Collaborate with the director to oversee program regulatory compliance and perform data collection and analysis for performance improvement.
• Develop and implement evidence-based policies and standards for the program and promote continued professional development.
• Maintain data and serve as subject matter expert for the program, including QA activities.
• Promote the organization's mission and service standards; may float to other departments or be on-call as needed.
