Qualifications

• Bachelor's Degree in Nursing required.
• Current RN license in the state of practice.
• BLS-HCP certification required at hire or within 60 days with approval.
• Three years of RN experience required.

Responsibilities

• Coordinate and guide patient care across diagnosis through follow-up.
• Participate in patient care rounds.
• Collaborate with director to ensure regulatory compliance and performance improvement.
• Develop and implement evidence-based policies and standards for the program.
• Maintain data and serve as SME for the program and QA activities.
• Support mission and values of the organization.
• May float to other departments as needed.
• May be on-call during scheduled shifts.
