Qualifications

• Bachelor's degree in nursing required.
• Current RN license in the state of practice.
• BLS for Healthcare Providers certification on hire or within 60 days.
• Three years of relevant RN experience.

Responsibilities

• Coordinate treatment processes from diagnosis through follow-up.
• Discuss program with physicians and nurses and ensure care follows evidence-based guidelines.
• Provide education to patients and families during hospital stay and after discharge.
• Actively participate in patient care rounds.
• Develop and implement evidence-based policies and standards for the program.
• Maintain data collection and analysis for performance improvement.
• Promote the program's mission and values; perform other duties as assigned.
• May float to other departments as needed; may be on-call during scheduled shifts.
