Responsibilities

• Coordinate treatment processes from diagnosis through follow-up.
• Educate patients and families during the hospital stay.
• Actively participate in patient care rounds.
• Review care for evidence-based guidelines and ensure regulatory compliance.
• Develop and implement policies and standards for the program.
• Maintain program data and serve as the subject matter expert.
• May float to other departments as needed.
• May be on-call during scheduled shifts.
