Job summary

Intermountain Health seeks a Clinical Program Coordinator in the Trauma department at Good Samaritan Hospital. You will guide patients through the care continuum, coordinate with physicians and care providers, and educate patients and families during the hospital stay and after discharge.

Requirements include RN licensure in the state, a Bachelor's in Nursing, BLS-HCP, and at least three years of RN experience. Prior trauma performance improvement experience preferred.

Qualifications

• Bachelor's degree in Nursing required
• Current RN license in state of practice
• BLS-HCP certification required on hire or within 60 days
• Three years RN experience required

Responsibilities

• Coordinate treatment processes from diagnosis through follow-up and educate patients and families.
• Participate in patient care rounds and ensure care meets evidence-based guidelines.
• Review charts and data, support performance improvement activities.
• Develop and implement evidence-based policies and standards for the program.
• Maintain data on the program and serve as subject matter expert.
