Job summary

Intermountain Health’s Trauma program seeks a Clinical Program Coordinator at Good Samaritan Hospital in Lafayette, CO. You will guide patients and families through the care continuum, coordinate with physicians and nurses, and support education during hospitalization.

The candidate must hold a Bachelor’s in Nursing, an active RN license in CO, BLS-HCP certification, and at least three years of RN experience, with trauma-focused improvement background preferred.

Qualifications

• Bachelor's Degree in Nursing
• Current RN license in the state of practice
• BLS-HCP certification either upon hire or within 60 days with approval
• Three years relevant RN experience

Responsibilities

• Coordinate program and ensure care aligns with evidence-based guidelines.
• Educate patients and families during hospital stay and serve as a resource after discharge.
• Participate in patient care rounds.
• Collaborate to develop and implement policies and standards for the program.
• Maintain and analyze data; act as subject matter expert for the program.
• Promote mission and values of the organization; may float to other departments as needed.
