Job summary

Intermountain Health in Colorado’s Good Samaritan Hospital seeks a Clinical Program Coordinator to assess, plan, implement and coordinate treatment processes across the patient care continuum. You will guide patients through the collaborative care model with physicians and nurses and provide educational support during the hospital stay and after discharge.

The role supports the trauma department, participates in rounds, reviews charts, analyzes data for performance improvement, and helps develop

Qualifications

Bachelor's degree in Nursing
Current RN license in the state of practice
BLS-HCP certification at hire or within 60 days
Three years of RN experience

Responsibilities

Coordinate program and ensure care meets evidence-based guidelines
Provide education to patients and families during hospital stay and after discharge
Participate in patient care rounds
Oversee data collection and chart reviews for performance improvement
Develop and implement evidence-based policies and standards
Maintain quality assurance for the program
Promote the mission and values of Intermountain Health
May float to other departments as needed
May be on-call during scheduled shifts
