Job summary

Allegheny Health Network is seeking a licensed Master’s level Social Worker to join the Care Management team. This role collaborates with providers, health plans, and community resources to coordinate care and address social determinants affecting health outcomes.

You will assess patients, plan safe transitions from inpatient settings, educate families about post-discharge options, and participate in department activities while upholding privacy and ethical standards.

Qualifications

• Master's degree in Social Work (MSW) required.
• PA LSW and/or LCSW required.
• Background clearances: Act 34, Act 33 and Act 73 (FBI fingerprinting) required.
• Experience in hospital/health care setting preferred.

Responsibilities

• Contributes to and/or completes initial and ongoing comprehensive assessment.
• Coordinates safe transition plans for inpatient/ED patients and connects with post-discharge resources.
• Educates patient and family/caregiver on level of care post discharge and access to community support.
• Provides crisis interventions for abuse, mental health, substance use, and related scenarios.
• Promotes professional growth and participates in department/hospital/system committees.

Master's degree in Social Work

PA LSW and/or LCSW

Act 34 Criminal Background Clearance Certificate

Act 33 Child Abuse Clearance Certificate

Act 73 FBI Fingerprinting Background Clearance Certificate
