Qualifications

• Minimum hospital/healthcare experience.
• MSW with LSW and/or LCSW licensure required (within specified timelines).
• PA LSW and/or LCSW required (Incumbents in the role on or before 04/21/2025 have 12 months from hire date to obtain).
• Act 34 Criminal Background Clearance Certificate
• Act 33 Child Abuse Clearance Certificate
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Responsibilities

• Contributes to and/or completes initial and ongoing comprehensive assessment. Coordinates care with the interdisciplinary team referencing social determinants of health.
• Collaborates to provide safe transition plans for discharge and connects with post-discharge resources.
• Educates patient and family/caregiver on levels of care post discharge and community supports.
• Provides counseling on treatment decisions and end-of-life issues and drives goals of care.
• Promotes professional growth through certification, mentoring, and department involvement.
• Supports department goals and performs other duties as assigned.
