Job summary

Allegheny Health Network is seeking a social work professional to coordinate care, advocate for patients and families, and collaborate with a wide care team. You will focus on social determinants of health to improve transitions, outcomes, and the patient experience across inpatient and post-acute settings.

The role requires a Master’s in Social Work with PA licensure or time to obtain, hospital experience, and a commitment to family-centered care.

Qualifications

• Master's degree in Social Work required.
• PA LSW and/or LCSW licensure or ability to obtain within 12 months.
• Hospital or health care setting experience preferred.

Responsibilities

• Contributes to and/or completes initial and ongoing comprehensive assessment.
• Provides interventions and implements recommendations after engaging patients and families.
• Focuses on social determinants of health to coordinate care across the continuum.
• Collaborates to ensure safe transition plans for inpatient and post-discharge care.
• Educates patient and family about level of care after discharge and community resources.
• Advocates through communication with care team, payers, and post-acute partners.
• Provides crisis interventions for abuse, mental health, and safety concerns.
• Promotes professional growth through mentoring and committee participation.
• Supports department goals and performs other duties as assigned.
