Job summary

Allegheny Health Network seeks a skilled social worker to provide care management, psychosocial assessments, and discharge planning across inpatient and outpatient settings. You will collaborate with a multidisciplinary team to support patient independence and community adjustment.

The role requires an MSW and PA-licensed LCSW/LSW with at least 5 years of healthcare social work experience, plus clearances (Act 34/33/73). Collaboration and crisis intervention are key components of daily duties.

Qualifications

• MSW is required by the role.
• LCSW or LSW licensed in Pennsylvania is required.
• Minimum 5 years of social work experience in a healthcare setting.
• Act 34, Act 33 and Act 73 background clearances required.

Responsibilities

• Complete and document psychosocial assessments and provide social work services and disease management.
• Develop and implement discharge plans for specific patient populations.
• Provide expert crisis intervention to patients and families.
• Advocate for patients/families to access community resources.
• Assist with unit coverage schedules and staff development.
• Educate patients and families and the care team on psychosocial factors related to illness.
• Performs other duties as assigned.
