Job summary

Allegheny Health Network is seeking a dedicated Care Manager to support patients and families by coordinating care across the continuum. You will work with an interdisciplinary team, advocates for patients, and focus on social determinants of health to improve quality of care and experience.

Qualifications

• Master's degree in Social Work.
• PA LSW or LCSW license.
• Experience in a hospital or health care setting.

Responsibilities

• Contributes to and/or completes initial and ongoing comprehensive assessment. Focuses on social determinants of health to assure successful coordination of care across the continuum.
• Collaborates to provide the safest transition plan for discharged patients and connects with post-discharge care providers and community resources.
• Educates patients and families about the most appropriate level of care post discharge and how to access community support.
• Provides counseling and crisis intervention related to treatment decisions and end-of-life issues, establishing goals of care.
• Promotes professional growth through certification, mentoring, and participation on department/hospital/system committees.
• Supports department-based goals contributing to the organization's success.
• Other duties as assigned.
