Job summary

Qualifications

• Minimum 2 years’ experience in hospital/multidisciplinary health care setting.

Responsibilities

• Completes psychosocial assessments, interventions and coordination of care.
• Provides crisis intervention and counseling to patients and families.
• Involves patient/family and team in discharge planning and post-acute resources.
• Educates patients and families about psychosocial factors related to illness.
• Engages in hospital social work initiatives and activities.
