Job summary

Allegheny Health Network seeks a social work professional to collaborate with an interdisciplinary team to coordinate care across the continuum. The role focuses on social determinants of health to improve patient outcomes and experience within a hospital setting.

Required are a Master’s in Social Work and PA LSW/LCSW licensure (or within the permitted timeframe). Clearances (Act 34/33/73) are required. Sign-on bonus of $10,000 is offered.

Qualifications

• Master's degree in Social Work is required.
• PA LSW and/or LCSW licensure required or obtainable within timeframe.
• Background clearances as specified (Act 34/33/73).
• Hospital or health care setting experience preferred.
• Strong coordination and communication with teams and patients.

Responsibilities

• Contributes to comprehensive assessments with patients and families.
• Plans safe transitions and discharge coordination with post-acute providers.
• Educates patient and family on level of care and community supports.
• Provides crisis intervention for abuse, mental health, and safety issues.
• Engages in professional growth and participates in committees.
