Responsibilities

• Contributes to and/or completes initial and ongoing comprehensive assessment and coordinates care with patients and families.
• Provides interventions and implements recommendations after engaging patients and caregivers.
• Educates patients and families on appropriate levels of care post discharge and connects with post-acute resources.
