Job summary

Community Solutions, Inc. (CSI) seeks a Residential Re-entry Specialist/Youth Residential Mentor to support youth in a residential setting and develop individualized plans with the team.

You will document progress, maintain records, and help residents reintegrate into their communities while ensuring safety and adherence to program rules. The role requires strong communication, problem-solving, and time-management skills, with a valid driver’s license and the ability to work a Sunday-Thursday

Qualifications

• High School Diploma plus 4 years experience in the social service or criminal justice field.
• Bachelor's degree plus one year experience in the social service or criminal justice field preferred. Education may be substituted with relevant experience on a one year for one year basis.
• Valid driver's license

Responsibilities

• Provide direct care and supervision to youth residents in a residential setting.
• Develop and implement individualized plans for each resident, in collaboration with other team members.
• Provide ongoing support and guidance to residents throughout their stay.
• Maintain accurate and up-to-date records of resident progress and behavior.
• Ensure that all residents receive the necessary support and resources to successfully reintegrate into their communities.
• Conduct employment, job search and home confinement spot checks on each shift.
• Conduct common area, room searches and perimeter searches as assigned and document results.
• Conduct pat/pocket searches on all clients entering the facility and document results.
• Complete house counts every hour from Midnight to 8am and every 2 hours from 8am to 10pm and document results.
• Transport Clients as needed in a Company vehicle.
• Conduct monthly emergency drills.
• Secure urines from same sex clients and complete all required paperwork and electronic documentation.
• Supervise all client visitation. Sign all visitors and contractors in and out of the facility.
• Complete intake on all new arrivals prior to the end of your shift. Facilitate client orientation as assigned.
• Facilitate all client movement in and out of the facility.
• Monitor clients self-administration of medication.
