Job summary

Health Here is seeking a Young Adult Housing Case Manager to support youth experiencing or at risk of homelessness in Everett, WA. You will conduct intakes, assess housing needs, and develop service plans with an emphasis on trauma-informed care.

Responsibilities

• Managing a small caseload and conducting intakes and housing assessments
• Developing individualized service and housing stability plans
• Providing trauma-informed, strengths-based support and teaching independent living skills
• Coordinating access to benefits, education, employment, and healthcare
• Partnering with landlords, community agencies, and internal teams to secure and maintain safe housing
• Maintaining accurate documentation and adhering to funder and agency requirements
