Job summary

Pathways to Housing DC seeks a Youth Case Manager for Montgomery County to provide trauma-informed case management and advocate for youth aged 16-24. The role emphasizes holistic support, collaboration with a multidisciplinary team, and connecting youth to housing, education, and employment resources.

Responsibilities include intake assessments, referrals, and documenting outcomes while operating at the MoCo Reconnect Drop-in Center during business hours.

Qualifications

• Bachelor's degree in social work or social service or 5 years of direct social service coordination.
• Experience with youth and young adults, especially vulnerable populations.
• SOAR experience preferred.
• Two years of related human services experience with Medicaid resources.
• Excellent written and oral communication skills; ability to communicate with individuals with varied cognitive abilities.
• Strong computer skills including Microsoft Word, Excel, Outlook, Teams, and 365 programs.
• English/Spanish strongly desired.

Responsibilities

• Building transformative relationships and providing comprehensive case management.
• Coordinating with community agencies to facilitate housing, education, and employment goals.
• Advocacy, crisis intervention, and ongoing support for youth.
• Maintaining records, reports, and program documentation.
