Job summary

Urban Peak Denver is seeking a Housing Case Manager to supervise youth on-site and at off-site activities, conducting assessments and crisis counseling. The role emphasizes trauma-informed approaches, data collection, and collaboration with partners to secure safe housing. Requires advanced degree or relevant experience with homeless youth.

Position is full-time in Denver, with hourly pay of $24.50. Applicants must pass background checks and TB screening, and have strong data management skills

Qualifications

• Must have advanced degree or equivalent experience in youth services.
• Experience with trauma-informed care and harm reduction principles.
• Ability to work with diverse youth and families while maintaining confidentiality.

Responsibilities

• Meet with assigned youth to obtain history and perform assessments.
• Provide trauma-informed guidance and therapy-based support to youth.
• Develop individual service and aftercare plans to achieve goals.
• Maintain accurate client files and document services in HMIS/Salesforce.
• Collaborate with partner agencies and families to secure housing.
• Deliver crisis intervention and on-site/off-site supervision as needed.
