Job summary

Urban Peak in Denver provides outreach to youth experiencing homelessness, connecting them with shelter and services to promote self-sufficiency. The role emphasizes trauma-informed care and collaboration with shelters, police, and public health entities to support youth facing complex challenges.

The Outreach Worker conducts street outreach, manages basic needs, and facilitates access to medical respite, housing, and community resources, ensuring safety and dignity for youth under 25.

Qualifications

• Master’s degree in social work, psychology, or related field with minimum 2 years’ treatment-oriented or case management experience OR bachelor’s degree with 3 years’ experience with youth
• Experience in trauma-informed care or willingness to adopt this approach
• Ability to work with diverse populations including LGBTQ youth and high-need individuals

Responsibilities

• Actively represent Urban Peak in the community to educate about homelessness and advocate for services
• Collaborate with shelters to connect homeless youth with housing and resources
• Maintain client records, assessments, and service plans in HMIS
• Provide case management, referrals, and follow-up to stabilize youth housing
• Collaborate with police, public health, and city partners to coordinate services
