Job summary

Urban Peak is seeking a Housing Case Manager in Denver to supervise youth on-site and at off-site activities, perform case management, and facilitate group work with a trauma-informed approach. You will collect data, maintain files, and coordinate services to support housing stability for youth.

Requirements include an advanced degree with relevant experience; CAC II preferred. Proficiency in Salesforce and HMIS is expected, with strong communication and confidentiality skills for a diverse

Qualifications

• Master's in social work or related field with minimum 2 years treatment-oriented or case management experience.
• Alternative: bachelor's in behavioral sciences with 3 years youth experience; or 5 years FT youth homelessness work.
• CAC II certification is preferred.

Responsibilities

• Meet with assigned youth to obtain history and complete assessments.
• Provide trauma-informed guidance and support to develop service plans.
• Develop and monitor individualized service and aftercare plans.
• Conduct weekly counseling sessions to review progress and adjust plans.
• Collaborate with parents/guardians and community agencies for service delivery.
• Accurately document services and maintain client files in HMIS.
