Job summary

The WorkPlace is seeking a Case Manager for the Platform to Employment Youth program in Bridgeport, CT. You will provide trauma-informed, individualized support to disconnected and at-risk youth aged 18-26, with travel to Norwalk, New Haven, Waterbury, and Hartford to engage participants and partners.

Based in Bridgeport, you will develop Employment Plans, coordinate wraparound services, and guide youth toward employment, education, or vocational training goals.

Qualifications

• Bachelor's degree or equivalent from a four-year college in Human Services, Education, or Business Management, and/or three years of related experience.
• Experience with case management and youth services preferred.
• Spanish language ability is desirable.
• Proficiency in Microsoft Office and data management systems.

Responsibilities

• Prepare and maintain case files with client information, services provided, and related documentation.
• Provide one-on-one case management and mentorship to disconnected and at-risk youth aged 18-26.
• Address barriers such as housing instability and mental health needs.
• Develop Employment Plan & Service Plans with short- and long-term objectives and monitor progress.
• Identify referrals to community services to support job placement and education.
• Monitor client progress and maintain program data for reporting.
• Represent the program on committees and coordinate outreach with community partners.
