Job summary

Community Solutions Inc. is seeking a Full-Time, On-Site Case Manager for our Bridgeport AIC program—the Alternative in the Community program. You will facilitate intake, assessments, case records, and discharge planning in a trauma-informed, community-based setting.

The role supports clients awaiting trial or on probation/parole, using Motivational Interviewing and evidence-based curricula. A Bachelor’s degree and one year of case management experience are required; travel up to 25% may be

Qualifications

• Bachelor’s degree and one year experience in case management or similar field; Education may be substituted with experience on a year-for-year basis.

Responsibilities

• Collect intake information within 5 days of referral and enter into CSI data systems.
• Perform client needs assessments within 5 days of intake and document in CSI/CSSD systems.
• Initiate clients into group within 15 business days from referral.
• Develop and document individual service plan goals with clients within 5 days of intake and review bi-weekly.
