Job summary

Recovery Network Of Programs is seeking a dedicated professional to provide client-centered case management and recovery support in Bridgeport, CT. The role involves conducting individual sessions, completing assessments, developing recovery plans, and coordinating housing and community resources.

The position requires a Bachelor’s in behavioral sciences and at least two years of counseling experience, with strong advocacy and bilingual abilities preferred.

Qualifications

• Bachelor's degree in the behavioral sciences and two (2) years experience in counseling preferred; or equivalent combination of education and experience.
• Co-occurring competencies and or credentials are preferred.

Responsibilities

• Conduct individual sessions with clients including in-person, office and home visits to assess and record medical, psychological, and social factors contributing to client's situation.
• Assist the Consumer with completing the HERO Participant agreement and identifying service and support needs.
• Complete HERO Case Management Assessment within 30 days of admission and every 6 months thereafter to help the Consumer identify resources and supports.
• Develop and implement a recovery plan with each Consumer addressing housing, income, benefits, health, and community supports.
