Job summary

The Connection in Bridgeport, CT is seeking a Case Manager 2 to deliver comprehensive case management services within the SHP program, focusing on housing stabilization for chronically homeless women and families in Bridgeport. You will conduct home- and community-based assessments, coordinate resources, and support participants toward stable housing.

Requirements include an Associate’s Degree, 2+ years of direct service and case management experience, a valid driver’s license, and reliable

Regular, predictable attendance required.

Responsibilities

delivering comprehensive case management services to clients in their homes and in their communities
conducting intake assessments
assisting youth in locating available apartments for rent
working in partnership with participants to develop and implement individual service plans that will progress them to stable housing
liaison with treatment providers and community resources
providing transportation to participants as needed
developing money management skills with caseload
developing employment readiness skills with caseload
liaison with landlords and property management staff to support participant’s continued tenancy
data entry and documentation
caseload size will vary between 8-12 cases at any given time
