Job summary

New Reach, Inc. seeks a Clinical Case Worker to provide core clinical assessment and case management for shelter residents in New Haven. The role includes developing housing goals, coordinating mental health and addiction referrals, and conducting biopsychosocial evaluations.

The position is in-person, with a Monday–Friday schedule, emphasizing confidentiality, teamwork, and trauma-informed care to support residents’ housing stability and well-being.

Qualifications

• Masters in Clinical Social Work or a related field is required.
• Two years of experience with homeless adults or those living in poverty preferred.
• Experience with homeless populations, HIV/AIDS preferred.

Responsibilities

• Develop treatment plans for residents entering Martha's Place.
• Provide referrals to mental health and addiction services and coordinate care.
• Conduct biopsychosocial assessments and safety planning.
• Lead Psychoeducational groups twice weekly.
• Supervise MSW interns and participate in trauma-informed committees.
• Maintain client records and complete monthly reports.
