Job summary

The WorkPlace is seeking a dedicated Case Manager for the Veteran Program in New Haven County. Based in New Haven County, you will provide personalized case management, assist with job readiness, and coordinate wraparound services to homeless and at‑risk Veterans.

Travel across New Haven County is required to engage participants and partners. The role emphasizes a strengths-based, culturally competent approach and collaboration across sites to support long‑term employment or education outcomes.

Qualifications

• Bachelor's degree or equivalent from a four-year college in Human Services, Education, or Business Management, and/or three years of related experience and/or training; or equivalent combination of education and experience.
• Strong interpersonal and organizational abilities are required.
• Proficiency with Microsoft Word, Excel, PowerPoint, and Access for documentation and reporting.
• Ability to work independently and with teams across multiple sites.
• Spanish language reading/speaking ability is desirable.

Responsibilities

• Prepare and maintain records and case files with clients' information and services.
• Provide one-on-one case management and mentorship to Veterans.
• Address barriers such as housing instability, mental health needs, and justice involvement.
• Develop Employment and Service Plans and monitor progress.
• Identify referrals to strengthen job placement and education opportunities.
• Maintain data and document program activities for reporting.
