Job summary

Emergence Health Network is seeking a Wraparound Facilitator to coordinate intensive case management using wraparound planning. The role emphasizes collaboration with families, youth, and community partners to deliver individualized, culturally competent supports.

The incumbent will ensure compliance with HHSC/Medicaid/NWIC, develop care plans, and facilitate team meetings, while maintaining trauma-informed, strength-based practices. This position is full-time and based in El Paso, TX.

Qualifications

• Bachelor's degree in Psychology, Social Sciences or related behavioral field; plus two years of direct casework/case management experience.

Responsibilities

• Deliver intensive case management using wraparound planning per Texas standards.
• Maintain compliance with HHSC, Medicaid and NWIC training requirements.
• Meet care standards and timelines, update plans of care every 90 days or as needed.
• Submit 95% of critical incident reports within 72 hours or three business days.
• Schedule 95% of emergency child and family team meetings within 72 hours to 7 business days.
• Provide high-fidelity wraparound services in a trauma-informed, family-driven approach.
