Job summary

Bluebonnet Trails Community Services is seeking a Wraparound Facilitator to provide high-fidelity, family-driven wraparound facilitation and care coordination under supervision. The role partners with youth, families, schools, and providers to develop care plans and monitor outcomes, ensuring services align with YES Waiver requirements.

The position requires a Bachelor’s in Human Services, Texas driver’s license, and experience in wellness and recovery support.

Qualifications

• Bachelor’s degree in Human Services required.
• Strong understanding of mental health diagnoses and treatment.
• Experience in wellness and recovery support in mental health, substance use treatment, and/or crisis intervention preferred.

Responsibilities

• Coordinate services across multiple systems and monitor progress toward outcomes.
• Facilitate Child and Family Team meetings and develop Wraparound Plans of care.
• Advocate for youth and families, ensure continuity of care and timely documentation.
• Maintain documentation in the electronic health record and manage multiple priorities.
• May include evening hours to accommodate youth and families.
