Job summary

The Harris Center for Mental Health and IDD is seeking a High Fidelity Wrap Around Facilitator to join the TXFFPS team in Houston. This role provides direct intensive case management using high-fidelity wraparound practices and requires extensive community collaboration with DFPS and Juvenile Courts.

You will conduct assessments, help formulate and authorize wraparound plans, and train families on accessing care while traveling to client homes and field locations as needed.

Qualifications

• Bachelor’s degree in human services required.
• Minimum of one year experience in mental health preferred.
• Ability to collaborate with DFPS and courts.
• Strong communication, documentation and teamwork skills.
• Willingness to travel to client homes and field locations.

Responsibilities

• Collaborates with DFPS and courts to develop the best High Fidelity Wraparound plan.
• Performs clinical assessments to identify service needs and progress.
• Writes and authorizes wraparound plans with LPHA signature.
• Ensures services meet eligibility criteria and are authorized.
• Provides skills training and rehab services addressing mental health needs.
• Responds to crises to reduce symptoms and prevent hospitalization.
• Educates families about youth needs and access to care.
• Monitors progress toward treatment goals.
