Job summary

Necco is seeking a Wraparound Facilitator to empower families as decision-makers in their planning process. You will lead the family team, conduct assessments, and provide intensive case management across home and community settings, traveling as needed.

The role emphasizes a client-centered, trauma-informed approach, use of CANS, and collaboration with a multidisciplinary network to advance family goals and outcomes.

Qualifications

• 21 years of age
• Bachelor’s degree; Master’s preferred in human services.
• Licensed Social Worker or eligible for licensure in West Virginia preferred, not required
• Minimum of 2 years experience working with families and/or children with emotional or behavioral problems
• Valid driver’s license
• 100/300/100 Auto Insurance Coverage
• Training and Travel willingness
• Excellent organizational skills
• Excellent written and oral communication skills
• Successful completion of all required criminal background checks
• Must possess the skills to navigate and execute software programs (e.g Microsoft Office, Database, Electronic Health Record, Dashboard)

Responsibilities

• Facilitation: Maintain a caseload of no more than 8-10 cases; lead the family team; provide assessments and intensive case management; oversee wraparound meetings; transport clients and travel between locations as needed.
• Program Development: Implement and follow the 10 Principles of the Wraparound; ensure the four phases are followed; maintain records and invoices; oversee team documentation and contracts.
• Family Planning: Client-centered, trauma-informed practice; use CANS; conduct weekly face-to-face meetings and monthly family team meetings; build a multidisciplinary support team; ensure plans progress.
• Corporate Citizen: Practice Ruthless Pragmatism; engage in peer feedback; embody Necco culture; uphold virtues Humble, Hungry, Smart; drive performance scorecard; follow meeting structure.
