Job summary

Bluebonnet Trails Community Services seeks a Wraparound Facilitator to provide high-fidelity, family-driven Wraparound facilitation and care coordination under supervision. You will partner with youth, families, natural supports and providers to develop and monitor care plans and coordinate services across systems.

The role emphasizes youth-guided services, crisis prevention and collaboration with multidisciplinary teams, with evening hours possible to meet family needs and enrollment in the YES

Qualifications

• Bachelor’s degree in Human Services as required.
• Strong understanding of mental health diagnoses and treatment.

Responsibilities

• Coordinate Wraparound services across multiple systems and monitor outcomes.
• Develop and monitor individualized Wraparound Plans of care and attend Child and Family Team meetings.
• Maintain timely documentation in the electronic health record and ensure compliance with standards.
