Job summary

Hoyleton Youth and Family Services seeks a Wraparound Care Coordinator to design and deliver strength-based services for youth and families referred to Wraparound. The role collaborates with youth, family, guardian, caseworker, and DCFS, delivering culturally competent and trauma-informed facilitation.

Flexibility is needed for evenings/weekends and travel to agency sites. Requires a Bachelor’s degree in a relevant field and at least three years of care coordination or related experience.

Qualifications

• Ability to manage multiple priorities and meet deadlines.
• Strong written and verbal communication with youth and families.
• Experience collaborating with multidisciplinary teams.
• Familiarity with trauma-informed approaches and care planning.

Responsibilities

• Evaluate referral info, complete discovery and CANS assessments at admission and discharge.
• Develop Crisis and Wraparound Plans with youth, family, and caseworker.
• Conduct weekly home visits and participate in monthly team meetings.
• Coordinate services using flex funding and ensure timely reimbursement.
• Deliver services in a culturally responsive and trauma-informed manner.
