Job summary

Family & Children’s Services seeks a Youth Care Coordinator to deliver targeted support and coordinate Systems of Care (SOC) for trauma-impacted youth and families. You will work in home and community settings, wrapping services around the whole family to strengthen home and school life.

The role requires at least one year of relevant experience, appropriate mental health training or certification, and a valid driver’s license to travel between locations.

Qualifications

• Minimum of one-year work with juveniles in the mental health field or related social service field.
• Specialized training and/or certification in mental health services or related field (e.g., Juvenile Case Management Certification) may be required.
• Ability to see, hear and speak clearly to interact with staff, consumers, family members, administration and the community.
• Must possess a Driver's License and use personal automobile to travel to locations other than primary office.

Responsibilities

• Provide targeted support services and youth-centered Systems of Care (SOC) in home and community settings.
• Coordinate mental health and community services aligned with trauma-informed care for youth and families.
• Engage families in treatment options and wraparound services to strengthen home, school, and community life.
• Assist with transportation of clients as needed; mileage reimbursement is provided.
