Health Connect America is seeking a qualified Therapist to provide high-quality behavioral health services to clients in a North Carolina/South Carolina community-based setting. Qualifications

• Master's Degree in Social Work, Counseling, or a related field required.
• Must hold a LMSW, LMFT-A, or LPC-A license.
• Experience conducting behavioral health assessments, treatment planning, and therapeutic interventions preferred.
• Strong knowledge of evidence-based treatment approaches, trauma-informed care, and crisis intervention practices.
• Excellent communication, documentation, collaboration, and relationship-building skills.
• Ability to work effectively within a multidisciplinary treatment team while managing an active caseload.

Responsibilities

• Conduct biopsychosocial assessments and utilize evidence-based assessment tools to identify client strengths, needs, and treatment recommendations.
• Develop individualized treatment plans with measurable goals and objectives in collaboration with clients, families, and multidisciplinary treatment teams.
• Provide individual, family, and group therapy using evidence-based interventions to address concerns such as anxiety, depression, trauma, behavioral challenges, and other mental health conditions.
• Monitor client progress, maintain ongoing communication and follow-up, and adjust treatment interventions to support positive outcomes.
• Provide crisis intervention, safety planning, and referrals to additional resources and support services as needed.
• Maintain timely, accurate clinical documentation while ensuring compliance with organizational policies, payer requirements, accreditation standards, and state regulations.
