Hope and Healing Survivor Resource Center in Akron, OH seeks an Access to Care Manager to oversee care coordination for survivors of domestic violence, sexual violence, and human trafficking. The role includes direct case management, advocacy, and staff supervision, ensuring timely, survivor-centered services.

The position requires a Bachelor’s degree in social work or related field, Ohio LSW eligibility, and prior supervisory experience.

Formación

• Bachelor’s degree in social work or a related human services field required. Current Ohio LSW or eligibility for Ohio social work licensure required
• Prior supervisory experience required
• Experience providing services to individuals impacted by trauma, violence, and other barriers to safety and stability
• Maintain valid Ohio driver’s license and ability to meet agency requirements for driving insurability.
• Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Investigations and Federal Bureau of Investigations).

Responsabilidades

• Oversees Access to Care Programming which includes case management services, advocacy in medical and other settings, and outreach and training efforts to meet the unique needs of a variety of populations
• Maintains an individual caseload to assist clients whose lives have been disrupted by sexual violence and human trafficking, which includes program intakes, goal planning, referral information, emotional support, crisis intervention, and other services as required
• Provides direct supervision to staff in a trauma-informed manner
• Maintains client and program documentation in compliance with agency procedure
• Assists with programmatic statistics, document creation, and evaluation
• Represent H&H at community meetings as assigned by supervisor
• Maintain confidentiality of information learned through one’s employment at the H&H
• Represent the mission and philosophy of the H&H at all times.
• Always adheres to Service Excellence standards.
• Complies with agency policies and procedures, federal and state requirements, CARF Standards and educational/certification/registry requirements.
• Performs other duties as assigned.
