Job summary

Hope & Healing Survivor Resource Center is seeking a Housing Case Manager in Akron, OH to provide trauma-informed case management and housing stabilization services for individuals and families experiencing homelessness or fleeing violence.

You will conduct safety and needs assessments, develop individualized service plans, assist with housing location, and support participants in achieving long-term goals related to financial stability, education and independent living skills.

Qualifications

• Bachelor’s degree in a relevant field or a minimum of five years of relevant housing services or case management experience.
• Preferred licensure or certification in social work or related field (LSW, LPC, CDCA).
• Knowledge of HUD housing programs, CoC requirements, and community housing resources.

Responsibilities

• Collaborate with Continuum of Care (CoC) partners and community agencies to coordinate trauma-informed rapid rehousing and transitional housing services.
• Conduct comprehensive needs assessments, housing inspections, and provide ongoing case management to assist survivors in securing and maintaining safe, stable housing.
• Develop individualized housing stability plans in collaboration with participants, providing ongoing support, advocacy, and connections to community resources.
• Establish and maintain collaborative relationships with landlords, housing authorities, property managers, and community partners to identify housing opportunities and address barriers to housing.
• Maintain accurate, timely, and confidential participant records and program documentation in accordance with agency policies, HUD regulations, and CoC requirements.
• Adheres to Service Excellence standards and complies with agency policies, federal and state requirements.
• Performs other duties as assigned
