Qualifications

• Associate degree in social/behavioral science or related field; 1 year social services experience.
• Working knowledge of community resources; proficient data entry in Word, Excel and Outlook.
• CPR/First Aid certification within 90 days; Naloxone (Narcan) training within 90 days; non-violent crisis de‑escalation training within 90 days.
• Valid Driver's License with clean record.

Responsibilities

• Conduct in-depth client interviews and orient individuals to the program.
• Develop individualized case plans and guide clients toward housing and services.
• Provide ongoing follow-up, review progress, and document notes in HMIS.
• Coordinate with housing providers to expedite placements and ensure stability.
• Assist with job readiness, applications, and resumes; transport as needed.
