Job summary

Health Connect America in North Carolina is seeking a licensed therapist to provide high-quality mental health services and collaborate with multidisciplinary teams to ensure comprehensive, client-centered care. You will develop treatment plans, deliver individual, group, and family sessions, monitor progress, and maintain accurate documentation while adhering to regulatory standards.

Qualifications

• Master’s degree in human services or a related field.
• Must hold one of the NC licenses LPCA, LCSWA, LMFTA or LCASA.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and create individualized treatment plans in collaboration with clients, families, and treatment teams.
• Deliver individual, group, and family sessions to address mental health challenges such as anxiety, depression, trauma, and behavioral disorders.
• Monitor client progress, adjusting treatment plans as needed, and provide crisis intervention when necessary.
• Maintain accurate, timely documentation of all interactions, treatment plans, and progress notes per company and regulatory standards.
• Collaborate with multidisciplinary teams and participate in treatment meetings to ensure holistic, coordinated care.
• Connect clients and families with additional resources and support services to address broader needs.
• Participate in outreach and community engagement to promote services and attract new clients.

Education
