City Care Inc in Oklahoma City is seeking a Resource Coordinator to assist guests experiencing homelessness with intake, assessments, and documentation recovery, linking them to wraparound services. This full-time role runs Monday–Friday, 11AM–7:00PM, offering meaningful work supporting neighbors and community restoration.

Qualifikationen

• Experience in case management or advocacy.
• Experience working with adults or children with disabilities.
• Pass pre-employment background check, driving record review, and drug screening.

Aufgaben

• Carry out the mission, vision, and values of City Care.
• Assist guests with shelter intake, assessments, and documentation collection.
• Complete HMIS data entry and other required documentation.
• Help clients obtain important documents such as IDs, Social Security cards, birth certificates, and proof of income.
• Educate incoming guests on shelter rules, expectations, and policies.
• Develop individualized plans with guests to address unmet needs and overall wellness.
• Connect guests to mental health, physical health, housing, food, and recovery resources.
• Provide case management support, referrals, and resource development.
• Collaborate with Oklahoma City Continuum of Care partners and community agencies.
• Maintain accurate and confidential client files and records.
• Conduct follow-up appointments and maintain consistent engagement with guests.
• Attend weekly staffing and community meetings as designated.
• Uphold professional confidentiality among guests, volunteers, interns, and staff.
