Job summary

City Care Inc. is seeking a Housing Stability Advocate in Oklahoma City to join a collaborative team delivering wrap-around support to individuals who recently experienced homelessness. You will conduct home visits, develop individualized service plans, and help residents set goals for housing stability.

This role emphasizes trauma-informed care, crisis response, and coordination with community partners to connect clients with housing, healthcare, benefits, and employment resources.

Qualifications

• Experience in case management, recovery services, behavioral health, or related fields.
• Experience working with adults and/or children with disabilities is preferred.
• Proficiency with data entry into electronic databases (HMIS preferred).

Responsibilities

• Carry out City Care’s mission, vision, and values.
• Build knowledge of community resources and develop relationships with service providers.
• Conduct regular home- and community-based visits to support housing stability and independent living.
• Partner with residents to set self-directed goals and monitor progress.
• Support residents in health, recovery, financial wellness, employment, education, and relationships.
• Provide case management, resource coordination, crisis intervention, advocacy, and direct support.
• Collaborate with City Care Property Management and programs to address tenancy concerns and improve communication.
• Coordinate with City Care programs and external providers to identify housing candidates and facilitate referrals.
• Collaborate with community partners to connect residents with resources and services.
• Navigate healthcare, behavioral health services, substance use treatment, public benefits, employment, and education.
• Promote resident independence through routines, self-advocacy, and self-determination.
• Identify and address barriers to housing stability and quality of life.
• Maintain accurate documentation of contacts, progress, referrals, and service plans.
• Provide transportation to appointments and resources within program guidelines.
• Respond to crises using trauma-informed, de-escalation, and person-centered practices.
• Participate in team meetings and multidisciplinary discussions about resident needs.
• Engage consistently with residents to exceed program expectations while staying compliant.
