Zusammenfassung

City Care Inc. is seeking an Advocate to assist guests experiencing homelessness with intake, assessments, and access to wraparound services in Oklahoma City. You will work trauma‑informed with vulnerable adults and families, helping them meet goals and connect with needed resources.

The role includes data entry, documentation, and collaboration with community partners while maintaining confidential records and following the Coordinated Entry System guidelines.

• HMIS experience preferred.

Aufgaben

• Carry out the mission, vision, and values of City Care.
• Assist guests with shelter intake, assessments, and documentation collection.
• Complete HMIS data entry and other required documentation.
• Help clients obtain important documents such as IDs, Social Security cards, birth certificates, and proof of income.
• Educate incoming guests on shelter rules, expectations, and policies.
• Develop individualized plans with guests to address unmet needs and overall wellness.
• Connect guests to mental health, physical health, housing, food, and recovery resources.
• Provide case management support, referrals, and resource development.
• Collaborate with Oklahoma City Continuum of Care partners and community agencies.
• Maintain accurate and confidential client files and records.
• Conduct follow-up appointments and maintain consistent engagement with guests.
• Attend weekly staffing and community meetings as designated.
• Uphold professional confidentiality among guests, volunteers, interns, and staff.
