Job summary

City Care Inc. in Oklahoma City is seeking a Resource Coordinator to assist guests experiencing homelessness with intake, assessments, and access to wraparound services. You will support clients in achieving goals, link with services, and ensure compliance with the Coordinated Entry System.

This role involves trauma-informed case management, data entry, and community resource coordination while collaborating with partners and maintaining confidential files.

Responsibilities

• Carry out the mission, vision, and values of City Care.
• Assist guests with shelter intake, assessments, and documentation collection.
• Complete HMIS data entry and other required documentation.
• Help clients obtain important documents such as IDs, Social Security cards, birth certificates, and proof of income.
• Educate incoming guests on shelter rules, expectations, and policies.
• Develop individualized plans with guests to address unmet needs and overall wellness.
• Connect guests to mental health, physical health, housing, food, and recovery resources.
• Provide case management support, referrals, and resource development.
• Collaborate with Oklahoma City Continuum of Care partners and community agencies.
• Maintain accurate and confidential client files and records.
• Conduct follow-up appointments and maintain consistent engagement with guests.
• Attend weekly staffing and community meetings as designated.
• Uphold professional confidentiality among guests, volunteers, interns, and staff.
