Job summary

You will implement behavior support plans, assist with daily living activities, organize structured therapeutic and recreational activities, transport residents to appointments, and maintain accurate records in the EHR while collaborating with a multidisciplinary

Qualifications

• High school diploma or equivalent required.
• Preferred: associate or bachelor’s in psychology, social work, or related field.
• Experience with individuals with emotional/behavioral/IDD in residential or community settings preferred.
• Flexibility to work across educational, residential, and group home settings.
• Strong communication and interpersonal skills; valid driver’s license.

Responsibilities

• Monitor and support residents' behavior using trauma-informed care and positive reinforcement.
• Implement PBSPs to promote skill-building and positive behavior changes.
• Provide direct supervision, mentoring, and crisis intervention as needed.
• Provide crisis intervention using PMT techniques.
• Teach and assist with ADLs, hygiene, meals, and household tasks.
• Organize structured therapeutic and recreational activities for socialization.
• Transport residents to medical appointments and community programs.
• Maintain accurate records and progress reports in the EHR.
• Communicate with team members and supervisors to ensure consistent support.
• Attend staff meetings and treatment planning sessions.
• Follow procedures for emergencies and mandated reporting.
• Maintain PMT training and apply de-escalation techniques as needed.
• Ensure compliance with policies, state regulations, and ethics.
